I am no longer on the provider list for Health Plan of Nevada, including Senior Dimensions and Sierra Health and Life.

I am not on the provider list for Universal Health Plans.
 
Your primary care physician may obtain an authorized referral but payment is not guaranteed by these plans.  You are responsible for any balance due.
