Confusion reigns with pharmacies sending refill requests to the wrong office. If your pharmacy says they faxed /called multiple times and I didn't respond, confirm with the pharmacy that they are contacting the correct office. Apparently some pharmacy computers can't track multiple office fax numbers for a single provider. Unfortunately this includes most mail order pharmacies as well as some local pharmacies.
You will be your own best advocate to make sure the pharmacy contacts the correct office. Because the two offices are independent of each other your prescription can only be authorized at the office where you are seen.

These phone numbers are also on your appointment card.
If you have not seen me since I left the Plumb Lane office I cannot provide refills until you are seen. You can ask your primary care provider for refills until I see you. You will need to be scheduled as a new patient because I will need the extra time with you to review your complete medical history for the charting system I now use.

Health history form for new patients
I have a health history form for new patients to complete. Please e-mail my office caruso.patient@gmail.com and the form can be sent to you. It is in Microsoft Word 2010 and can be completed electronically. If you cannot open a Word 2010 document please state so in your e-mail and I can send a PDF version for you to print.

Bike to work week
The official bike to work week starts tomorrow, May 11. The weather forcast looks ideal for this adventure. I took a practice ride today to the Renown Diabetes Center, a 9+ mile ride. It takes me about 45' going slow enough the second half of the ride to not get overheated. I'm fortunate that I can leave a set of work clothes at the office so I don't have to carry it with me on the bike. It's going to be a very good week.
If you have a bike, get out and ride. If biking to work isn't feasable then bike to the coffee shop, your friend's house, the park or just joy ride around the neighborhood. Go fast enough to get some exercise yet slow enough to enjoy the view. Happy pedaling!

On-line scheduling at Riggins Court
My electronic medical record company, Practice Fusion, has a system for you to schedule your follow up appointment electronically. You go to the web page, find an open spot and follow the instructions ro request that appointment. The major limiting feature is that it only goes out for three months and I'm now scheduled out to August. But as cancellations occur those spots are visible on the calendar you see. I am not going to schedule new patients by this system. Physician referral is needed for a new patient.

I am not on the provider list for Universal Health Plans.
Your primary care physician may obtain an authorized referral but payment is not guaranteed by these plans. You are responsible for any balance due.
